Required Documents for Registration:

The following documents must be completed, signed, and uploaded into your SportsConnect player profile. All documents will be reviewed by June 15th, 2026. You will be notified of any changes needed. Registration will not be approved, and you will not receive any equipment, until all paperwork is complete and full payment is received.

• Birth Certificate
• Player & Parent Code of Conduct/Contract (Download Form)
• Medical Release Form (Download Form)

• A physical examination is no longer required as part of the registration process this season. 

• School report cards are no longer required for registration. This is no longer a document submission requirement.  Academic standards still apply — athletes must maintain a 2.0 GPA or 70% average throughout the season to remain eligible.
